ACM Care of Peoria was our first venture into Illinois. Our local Social Workers and support staff work with clients to access resources and age successfully. We follow clients through hospitalizations, nursing home stays, and we continue to help them when home. We also manage several programs through the State of Illinois Department of Aging for clients who qualify.
